What is IVF?

In vitro Fertilization (IVF) is the most effective form of assisted reproductive technology. It is a process of fertilisation where an egg is combined with sperm “in vitro”, followed by the transfer of the embryo into the body of the expectant mother.

Stimulation of ovulation

It is carried out in order to obtain not one, as is most often the case in the natural cycle, but several eggs. During this stage, the patient is administered medications that stimulate the growth of follicles according to a special protocol. The presence of several eggs allows the embryologist to select the best of them for further fertilization. The duration of this stage is from 10 to 14 days.

In some cases, ovulation is not stimulated, and a woman’s only naturally mature egg is punctured during ovulation. The IVF program in the natural cycle is carried out in cases where a woman does not want to expose her body to the risks associated with hormonal stimulation; if hormonal stimulation is contraindicated for her; as well as in cases where hormonal stimulation, carried out in previous IVF attempts, did not give a sufficient number of eggs suitable for fertilization.

Fertilization by ICSI method

ICSI is an abbreviation of the English term Intra Cytoplasmic Sperm Injection, which literally translates as “the introduction of a spermatozoon into the oocyte cytoplasm”. When an egg is fertilized using the ICSI method, the spermatozoon is injected directly into the cytoplasm of the egg. For fertilization, the morphologically best and most mobile sperm are selected. With him, the procedure of immobilization and activation for the fertilization process is carried out. During the manipulation, the egg is fixed so that the injection needle does not touch the genetic apparatus of the egg.

After 16-18 hours, the effectiveness of fertilization is assessed. And if the embryologists determine that the fertilization procedure was not effective enough (one or three pronuclei are observed in the middle of the cell), the zygote – this is the name of the fertilized egg 16-18 hours after ICSI – is excluded from the IVF program. Embryologists closely monitor the development of a fertilized egg for five days. After this period, the best of the obtained embryos is either transferred to the uterine cavity of the future woman, or, if the NGS genetic diagnostic method is used, it is cryopreserved until the results of the analysis are obtained.

NGS (upon patient's request)

NGS (Next Generation Sequencing) is a preimplantation genetic screening that detects genetic abnormalities in embryos. It considerably improves the efficiency of IVF. Even before embryo implantation, it allows to detect all the known chromosomal pathologies, which approx. in 50% cases form the reason for the pregnancy failure during the IVF program. The accuracy of checking the embryo for chromosomal and monogenic diseases is 99.9%. The process is fully automated, which eliminates the impact of the human factor. At ISIDA clinic, more than 25% of IVF cycles are carried out using the NGS method. Since the introduction of this method, it has been used in more than 160 IVF cycles and more than 400 embryos have been analyzed.

Transfer of the embryo into the uterine cavity and pregnancy monitoring

This manipulation is carried out under the control of ultrasound, and it does not require anesthesia. Two weeks after the transfer of the embryo, a blood test is performed to determine the level of HCG (human chorionic gonadotropin) in the blood and evaluate the effectiveness of the program. A positive blood test for hCG indicates that the IVF program was successful.
The final confirmation of pregnancy is done with the help of ultrasound 4 weeks after the transfer of the embryo.

ICSI

ICSI – Intracytoplasmic Sperm into Egg Injection – is a method that allows to achieve egg fertilization and pregnancy even in the most severe forms of male infertility.

The ICSI procedure is carried out in an embryological laboratory, where, under the control of a microscope with the help of micromanipulators, a selected sperm of good quality is injected into the cytoplasm of a mature egg.

Surrogacy

Surrogate motherhood is a special procedure when the pregnancy is carried not by the future mother, but by the surrogate mother. There are two main types of surrogacy programs: with the use of germ cells of future parents and with the use of donor germ cells.

An IVF program is used to transfer embryos into the uterus of a surrogate mother. At the same time, the use of eggs of a surrogate mother for fertilization is not allowed.

The success rate of the IVF program depends on many factors: age, egg quality, sperm-related issues, medical diseases, cervix and uterine structure, etc. However, the combination of the work of ISIDA clinic specialists and the use of modern equipment provides good treatment results.
